This Hot Work Permit is required for any operation involving open flame or producing heat and/or sparks. This work includes, but is not limited to, welding, brazing, cutting, grinding, soldering, thawing pipe, torch-applied roofing, or chemical welding. 1.2. Hot Work Permit is valid for a single shift period only. 1.3. Before issuing the permit, the relevant permit issuer must inspect the area and ensure that the safety requirements have been implemented. 2. BEFORE PERFORMING HOT WORK 3.1.1. Equipment must be thoroughly inspected and found to be in good repair, free of damage or defects. 3.1.2. A multi-purpose dry chemical, portable fire extinguisher must be so located as to be immediately available, fully charged and ready for use at the work site. 3.1.3. A means of contacting the fire department (i.e. site telephone) must be available and accessible to fire watch person(s). 3.1.4. All combustible materials within a 10m radius of the work site are relocated or protected by approved welding blankets. 3.1.5. Hot work is not allowed to be performed near walls, ceilings or roofs of combustible construction unless protected / covered by fire resistant shields (fire retardant plywood, flameproof tarpaulin, metal, etc.). 3.2. DURING AND POST THE HOT WORK PERFORMED 3.2.1. Person(s) must be assigned to a fire watch during and for at least 60 minutes after all cutting/welding ceases. 3.2.2. Fire watch person(s) is to be supplied with multi-purpose dry chemicals, portable fire extinguisher and trained in its use. 3.2.3. A means of contacting the fire department (i.e., site telephone) must be available and accessible to fire watch person(s). 4. ATTESTATION The location where the work will take place has been EXAMINED BEFORE THE START of the hot work operations and all the appropriate precautions mentioned above taken. The work area and all adjacent areas to which sparks, and heat might have spread (including floor levels above and below and on opposite side of walls) were INSPECTED 60 MINUTES AFTER the hot works operations ceased for the day and were found to be fire safe.
